BCHealthCare has published a guide on home and community care. According to the guide:

This guide is for people who have difficulties coping with activities of daily living because of health-related problems or have palliative care needs. The guide includes general information about services available to help British Columbians live as independently as possible and information about end-of-life care. It also provides useful information for family and friends.
